Job Summary:
- To ensure that good working relationships are maintained within the multi-disciplinary team, and with external agencies, through effective and clear communication.
- To take a lead role in setting and maintaining standards of care within the Community Mental Health Team (OPHA CMHT), in consultation with the Team Manager and Service Manager
- To take a lead role in providing comprehensive, culturally appropriate, evidence based initial assessment of people referred to the OPHA CMHT, determining the specific care needs of the individual and positively managing risk indicators.
- To maintain an environment in which there is equal opportunity and anti-discriminatory practice.
- To ensure the safe and effective delivery of care to service users, utilising the knowledge and skills of staff, whilst recognising and working towards helping them manage their stress in this demanding area of work.
- To promote a positive image of the service to its users, their carers, family and partners, and through liaison with all other agencies.
- To support the Manager in maintaining clinical, operational performance
- To participate as an active member of the team, accountable for your practice, and work in partnership with other health care professionals, service users and others in the delivery of care.
- To identify Adult Protection and Child Protection issues and to implement procedures as required.
- To act as Safeguarding lead for the team, ensuring that all aspects of Safeguarding legislation and guidance are adhered to within the working practices of the OPHA CMHT, and highlighting to Managers were there are Safeguarding issues with any services users CMHT caseload.

- Degree in nursing
- NMC Pin Registered
