Job Summary:

  • To carry out comprehensive functional assessments for patients within the locality who are experiencing cognitive and memory impairment.  These assessments are necessary to inform potential diagnosis of Dementia.  All assessments and interventions are predominantly carried out in patient’s home environment.
  • To provide specialist occupational therapy to clients with complex needs using evidenced-based / client-centred principles to assess, plan, implement and evaluate interventions at all stages of the service user’s journey; cognitive and functional assessment to support diagnosis, advising on dementia design, home modification, assistive technology, therapeutic activities and cognitive rehabilitation.
  • To carry out comprehensive assessments and evidence-based interventions utilising the occupational therapy process.  Deliver personalised and goal-orientated rehabilitation programme to enable patient to remain at home, reduce care package needs and delay care home placements.
  • To actively develop and implement a framework of care, which reflects a holistic approach to service user needs, and the seven pillars of clinical governance.
  • To use evidence-based principles to apply clinical skills and reasoning in occupational therapy, assessment, treatment planning, intervention and evaluation which include individual and group orientated approaches.
  • To be a resource for the team, providing, occupational therapy professional guidance, and to lead on the evaluation and continued development of the occupational therapy service
  • To prescribe and order adaptive equipment and educate patients to use them safely.
  • To provide leadership and taking responsibility for the direct supervision for less clinically experienced staff, support staff and students including conducting Performance Development Review (PDR) as appropriate.

Essential Candidate Criteria:

  • Degree in Occupational Therapy
  • HCPC Registered 
  • Right to work in the UK
