Blackstone Recruitment are working with a NHS Trust in South West that are looking for a Band 6/7 Clinical Pharmacist for their hospital service. Training provided.
This is a Locum role, to start ASAP for a minimum of 3 months, with the view to be an ongoing contract.
KEY RESPONSIBILITIES/OUTCOMES:
Clinical:
- To provide a Clinical Pharmacy service to designated wards under the supervision of a Senior or Principal Pharmacist, assessing the suitability of prescribed medicines.
- To participate in the delivery of ward and dispensary based pharmaceutical services and maintain clinical and technical skills including:
- To implement a good framework of pharmaceutical care through effective drug history taking and documentation, review of patient medication, evaluating efficacy and toxicity of medication used, observing and reporting risk issues relating to the use of medications, when necessary.
- To work as a pharmacist, providing a dispensary service including accurately dispensing, checking medication and counselling patients regarding their medication as required.
- To provide information and advice on the preparation and administration of intravenous drugs.
- To demonstrate and communicate effective and expert clinical pharmacy knowledge and advice to healthcare professionals and patients.
- Practice in accordance with the Code of Ethics of the General Pharmaceutical Council (GPhC), professional standards of the GPhC and Royal Pharmaceutical Society of Great Britain / and within the Medicines Act (1968) and subsequent legislation.
- To undertake effective monitoring for the use of medication, adjusting dosages and route of administration through timely intervention and communication with medical, nursing and other healthcare professionals, when necessary.
- To liaise with carers, senior pharmacists, nursing or medical staff to optimise medicines management for patients, allowing best outcomes for medical interventions.
- To participate in weekend and Bank Holiday duties as required in the provision of cover for ‘out of hours’ Pharmacy services.
- To cover specialist areas, eg NNU / paediatrics, TPN, haemato-oncology and Medicines Information as required.
- To clinically screen, order and check chemotherapy.
- To clinically screen prescriptions for biologics and high cost drugs including homecare prescriptions.
- To ensure that prescribing is in line with formulary guidelines and prescribing policies
- To provide a clinical pharmacy service to AAU at weekends according to the rota
- To contribute to and adhere to Clinical Pharmacy Service standards.
- To liaise as appropriate with other Healthcare professionals within Primary care to improve communication across the interface to ensure continuity of care.
- To assist in the development of the clinical pharmacy service to each of the specialities, working with each on a rotational basis under the supervision of a Senior Pharmacist.
- To produce evaluated information for the Drug & Therapeutics Group to assist decision making on the inclusion of new drugs into the Formulary under the guidance of the Principal Pharmacist Medicines Management.
- To train and work as an Independent Prescriber according to Trust policies and within your identified scope of practice, if required.
Job Specifics
Banding: 6/7
Hourly Rate: Between £27 to £33 per hour (depending on banding and experience)
Working Hours: Monday to Friday. 08:45 to 5:15

Essential Criteria
- Master`s degree in Pharmacy (MPharm)
- Member of the General Pharmaceutical Council
- Right to Work in the UK
- Post graduate degree in clinical pharmacy/pharmacy practice or equivalent is desirable
- 12 months UK NHS hospital experience
- Experience as a clinical pharmacist on wards
