Banding: 7
Hourly Rate: £28 to £30 per hour
Working hours: Monday - Friday 37.5 hours (will consider part time)
Payment Method: Direct Engagement

Contract Length: 3 months minimum
Job Summary:
- Supporting the new frailty team.
- Supporting frail patients to discharge.
- Working in a multidisciplinary team.
Essential Candidate Criteria:
- Masters Degree in Pharmacy
- GPhC Registered as a Pharmacist
- Right to work in the UK
- Experience working in a Frailty team.
